MEDICAL DEVICE
Alidya is the first injectable solution for the treatment of cellulite, i.e., an injectable solution for specific medical use applied according to a protocol involving intradermotherapy, which addresses the physiological and para-physiological alterations linked to the origin and evolution of gynoid lipodystrophy (cellulite). It was developed by Prof. Pasquale Motolese and is patented worldwide. Indications: treatment of gynoid lipodystrophy (cellulite), post-liposuction treatment, post-intralipotherapy, lymphedema with or without lipoedema, alterations of subcutaneous adipose tissue.
The Alidya project aims to provide an assistive rather than merely functional therapy by using means to correct changes in the structure of the skin and subcutaneous tissues for lipodystrophy and hypotrophy. In reality, these changes manifest clinically as small negative depressions or indentations on the skin surface, leading to uneven and cracked skin.
Correcting these skin depressions or indentations on the skin surface is not possible with existing absorbent fillers available on the market. Attempts with polyactic acid have failed, as it proved to be highly reactive and profibrotic, leading to significant creation of scar tissue. In practice, it is very difficult to make corrections with gelatinous and highly viscous products that release microparticles.
Alidya, with its special formulation, is intended for superficial subcutaneous tissues and differs from other products because it also acts on deeper parts of the skin, such as the dermis. Alydia represents a new class of injectable amino acid-based agents similar to those already on the market and used for skin rejuvenation (Jalupro, Skinr..). The difference is that Alidya's formulation is better suited to the biological environment, whether it be a superficial subcutaneous part or a deeper part of the skin.

Alydia is a solution for injections into the outer subcutaneous part of the skin, containing rapidly absorbed components (within a few hours) that do not contain pharmacologically active substances.
Although there is no other product with a similar specific indication, there is a vast number of products with a similar composition that are used for the purpose of skin biorevitalization and biorestruction. The difference between these products and Alidya is that Alidya's formulation is adapted to osmolarity and pH values in different environments, not just those within the dermis.
Medical evaluation
Clinical research conducted on women aged between 28 and 65 years showed excellent tolerability of the injectable agents with no skin reactions of any kind. Furthermore, the initial research (35 women treated once a week for 7 weeks) showed a high degree of satisfaction among the interviewed women, with a desire to repeat the treatment as soon as possible.

Alidya – eliminates cellulite and provides permanent results
By definition, cellulite is a phenomenon that causes a chain of negative physical and psychological reactions.
The medical name for cellulite is "ADIPOSIS EDEMATOSA".
The development of cellulite is slow and proceeds in phases:
- Venous and lymphatic pathway phase: Altered microcirculation and small blood vessels spread in the deep tissue of the dermis;
- Edematous phase: The drainage system begins to function more slowly, and waste materials are retained within the interstitial space. This causes fluid to pass through the blood vessels into the tissues, leading to the development of edema. The interstitial space widens, suppresses its contents, and decreases drainage, creating a vicious circle;
- Proliferative phase: The irregularities that occur disrupt the regular flow of nutrients and oxygen. Malnourished cells undergo changes in number and size, glycosaminoglycan production increases, appears to be of a more viscous consistency, and elastic fibers harden, forming a reticulum with the appearance of shapeless adipocytes that collect in nodes on the surface;
- Sclerosis phase: Here, more palpable nodes are present, there is a reaction and creation of fibrotic scars that pull round formations of the dermis, causing orange peel skin, i.e., the characteristic appearance of cellulite.
Studies by Dr. Motolese published in the European Journal of Aesthetic Medicine Dermatology at the end of 2011 address the pathophysiological causes of cellulite formation. The most widespread hypothesis concerns the anatomical pattern of adipose lobules located in the outer area of the subcutaneous adipose tissue, which becomes inflamed and determines the initial stage (obstruction), increasing microcirculatory permeability. The identification of potentially toxic elements could be a cause of tissue fibrosis.
Three tissue biopsies taken from three different women showed traces of certain metals, thus it is presumed that this is due to the presence of toxic intra-adipose metals linked to tissue degeneration, which develops into cellulite.
The significance of cellulite refers to its aesthetic appearance, which is primarily observed in the female population, where changes in skin profile occur, especially in the lower limbs. Its nature is complex and controversial; therefore, despite limited scientific evidence, three pathogeneses are described:
- Circulatory disturbance causing microvascular pathways in the subcutaneous adipose tissue, linked to:
- Female hormonal factors, which have an impact on chronic tissue inflammation,
- Structural anatomical defect involving the presence of administrative fibrotic septa on the skin surface, with an angled orientation of adipose lobules (in men, the orientation is parallel to the skin surface).

During the research, biopsies were conducted on the anterolateral cellulite thighs of three women aged 33, 35, and 40, to determine the presence of "ferritin-like" matter in the extracellular matrix of the adipose tissue region. The degree of cellulite could be classified between III-IV.
Microscopic examination showed that the adipose tissue consisted of cells of different sizes surrounded by a large area of sclerosis, with a strong presence of iron and hemosiderin.
It is concluded that in the presence of these metals, tissue degeneration occurs through the creation of free radicals, peroxidation of lipid adipocyte membranes creating chronic inflammation, and tissue acidosis.
This enlargement will result from vascular permeability, which can be seen in cellulite, and iron will exert its toxic and degenerative effects on subcutaneous adipose tissue, causing an inflamed state, acidosis, cell degeneration, and necrosis. It will gradually arise and cause continuous damage.
Alidya treatment promotes the removal of chelated iron produced by EDTA and the amino acid buffer solution with sodium bicarbonate corrector to compensate for the alkaline environment and with it, the cellulitic tissue acidosis.

- D-glucopyranose: cyclic glucose generates the intermolecular reaction of the alcoholic carbonyl group, forming the pyranose form of glucose.
Effect: A mixture of amino acids, metal chelator, alkalized system connected with the extracellular space can create equilibrium, returning the adipose tissue structure to normal.
Usage:
- Intradermal: mesotherapy or 30G – 4mm needle, at least 7 applications per week, maximum 12 weeks
- After application, apply drainage
- Do not use cosmetics on the application area for at least 12 hours after treatment
- Do not expose to high heat and/or cold.
